These forms give us important background information about your medical and dental history, helping us provide care that is tailored to your needs right from the start.
If you are transferring from another dental office, you can also complete an authorization form so we can request your previous records.
You are welcome to fill out your forms at home before your appointment or arrive a little early to complete them in the office. If you have any questions, our team is always here to help.
