New Patient Intake Forms
We require all new patients to complete an intake form before their first appointment. You can download this form and bring it with you to your first appointment, or come 15 minutes early to fill it out at the clinic.
Patient information forms give us some additional information about your overall health history, dental history, and insurance information. It also informs us of any prior dental work or ongoing conditions such as TMJ disorder or periodontal disease, which may require our additional attention.

There is plenty of free parking, including accessible stalls, right outside our clinic.
